Community Action Wayne/Medina has funds from the Austin Bailey Health and Wellness Foundation and the North Canton Medical Foundation to provide free out-of-county medical transportation to Wayne County residents who meet the following guidelines:

Income must qualify (80% of the Wayne County area median income). The total income for everyone living in the home who are 18 or older is required.

—One person in the home: $3,204 per month or $38,450 for 12 months.

—Two people in the home: $3,663 per month or $43,950 for 12 months.

—Three people in the home: $4,121 per month or $49,450 for 12 months.

—Four people in the home: $4,575 per month or $54,900 for 12 months.

—Five people in the home: $4,942 per month or $59,300 for 12 months.

—Six people in the home: $5,308 per month or $63,700 for 12 months.

—Seven people in the home: $5,675 per month or $68,100 for 12 months.

—Eight people in the home: $6,042 per month or $72,500 for 12 months.

Documentation needed includes state-issued photo ID; proof of citizenship for all members of the home such as birth certificates, voter registration cards and Social Security cards; Social Security cards for all members of the home; and proof of income for the past 90 days from all sources for members of the home 18 or older (can use pay stubs, bank statements showing direct deposit, proof of direct deposits, letter from Social Security listing benefit amount).
